Older Adults · Dementia · Alzheimer’s
Evaluations for adults experiencing memory problems, confusion, or cognitive decline — distinguishing normal aging from conditions requiring medical attention.
Testing distinguishes between normal aging and conditions such as mild cognitive impairment (MCI), Alzheimer's disease, vascular dementia, Lewy body dementia, frontotemporal dementia, depression that mimics memory loss, TBI/concussions, and medication effects on cognition.
When testing is medically necessary, Medicare and most major insurance plans cover the cost. Contact us to verify your coverage before scheduling.
Early assessment allows identification of treatable causes, access to medications that may slow progression, and planning for the future while the person can participate in decisions.
Citizenship · USCIS · Se habla español
Are you, or is a family member, applying for U.S. citizenship but unable to meet the English language or civics requirements because of a medical or developmental condition? You may qualify for an exemption, but first you need documentation from a licensed clinical psychologist.
USCIS Form N-648 (Medical Certification for Disability Exceptions) requires an evaluation by a licensed provider who can certify that a medically determinable physical or developmental disability or mental impairment prevents the applicant from meeting the standard requirements for naturalization. Dr. Nuñez is experienced in completing these evaluations and providing the comprehensive documentation USCIS requires.
The evaluation typically includes a clinical interview (with a family member or caregiver present if helpful), a review of relevant medical records, and cognitive testing to assess memory, language, and overall functioning. The process is conducted in Spanish or English, based on your preference. A formal written report meeting USCIS standards is provided, along with the completed N-648 form.
Important: USCIS does not guarantee approval of any N-648 waiver. The evaluation documents the medical basis for the exemption request — the final decision rests with USCIS. Begin the process as early as possible, as there may be wait times for appointments and for USCIS to process your application.

Schedule as soon as you or your family notice concerning changes. Early evaluation can identify treatable causes, detect mild impairment before it progresses, establish a baseline for monitoring, and provide peace of mind if testing shows normal aging.
